In video games, False PHD is a high-risk, high-reward passive item added in The Binding of Isaac: Repentance expansion. It subverts the traditional mechanics of the game’s item system by transforming how consumable pills behave throughout a run. Core Mechanics

Immediate Bonus: It grants exactly one Black Heart immediately upon picking up the item.

While it forces your pill pool to become negative, it offsets this by adding powerful secondary effects whenever you consume a bad pill:
Stat Down Pills: Whenever you take a stat-reducing pill (e.g., Speed Down, Range Down), you suffer that stat penalty but permanently gain a Flat Damage Up bonus.

Rock Bottom: This item prevents your stats from ever decreasing. When paired with False PHD, you gain the Damage Up bonus from stat-down pills without suffering the actual stat penalty.
A Pony / White Pony: These active items lock your movement speed to a high baseline, effectively neutralizing the penalty of Speed Down pills while allowing you to harvest the damage bonuses.
